Home Health Physical Therapist – Bloomfield, New Jersey

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full‑time position in Bloomfield, New Jersey. The role requires an ongoing employment schedule and is available to qualified staff.

• Make home visits to adult and geriatric clients in designated geographic territories throughout Essex County.
• Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence‑based dressings and topical agents.
• Develop and implement individualized care plans that include manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the Physical Therapy care plan and collaborate with nursing in multidisciplinary care planning as appropriate.
• Educate and instruct clients, family members, or other representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations in a state‑of‑the‑art touch‑pad tablet.

• Current New Jersey Physical Therapist – PT license.
• Minimum of one year of recent work experience as a licensed Physical Therapist.
• Graduation from an APTA‑approved program or one accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

Base Pay will be negotiated based on qualifications and may range from $68.00 to $75.00 per point.
