Job Summary:

You will provide outpatient, recovery-oriented therapeutic services to adults, children, and families. Additionally, this position serves as a Peer Navigator assisting clients with identified legal needs through care coordination, advocacy, and linkage to medical leadership and partnering attorneys. The integrated role addresses clinical, social, and legal determinants of health to support long-term recovery and improved client outcomes.

What You Will Do:
  • Work with children, families, and adults on their personal treatment goals in order to help create a sense of stability and overall healthy functioning
  • Assess and evaluate clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer
  • Formulate psychosocial treatment plans, with relevant goals, behaviorally measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
  • Provide supportive services and evidence-based interventions, including case management, linkage to community resources, and inter-agency coordination of treatment
  • Document services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in cli services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated
  • Plan for, communicate, and document discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
  • Track and manage referrals related to the medical-legal partnership, ensuring timely follow-up and appropriate linkage to services.
  • Provide care coordination and system navigation support to clients with identified legal and social service needs, in collaboration with medical leadership, partnering attorneys, and community providers.
  • Prepare and submit required monthly reports to the state, including referral data, service outcomes, and program activity metrics, in alignment with SAMHSA and DMHAS reporting standards
What We Need to See:
  • Master's degree in a behavioral health discipline from an accredited college or university and documented knowledge of at least 1-2 years of clinical experience in the mental health field.
  • Must hold a valid Licensed Clinical Alcohol and Drug Counselor (LCADC) credential in addition to one of the following New Jersey professional licenses: (LAC, LSW, LPC, LCSW, LMFT)
  • Candidates holding an LSW, LAC, or LMFT must have a minimum of two (2) years of documented experience providing substance use disorder treatment or addiction-related services.
  • A valid driver's license and must be insurable by agency's automobile insurance for primary drivers.
