Hospice of New Jersey is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our compassionate nursing team. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey in the Bloomfield area.
Requirements include a current RN license, 1 year of RN experience (medical-surgical/acute care preferred), EMR proficiency, and a valid driver's license.
