At SYNERGY Home Care of Clifton–Secaucus, we are seeking a Clinical Operations Coordinator (LPN) to support client assessments, care documentation, and clinical coordination across our growing home care agency.
This role is critical to ensuring timely client onboarding, high-quality care planning, and operational efficiency. You will work closely with leadership and a Registered Nurse (RN) to support assessments and ensure compliance with New Jersey regulations.

Key Responsibilities:Client Assessments & Field Visits.
Conduct in-home client assessments and gather detailed clinical and environmental information.
Observe and document client condition, functional needs, and safety risks.
Identify changes in condition and upscale appropriately.
Prepare assessment documentation for RN review and approval.
Assist with development and updates of care plans.
Ensure all documentation is accurate, complete, and submitted in a timely manner.
Collaborate with schedulers and leadership to align caregiver assignments with client needs
Provide input on appropriate caregiver matching based on assessment findings
Support onboarding of new clients through efficient assessment turnaround
Compliance & Quality Assurance
Ensure all documentation meets NJ state requirements and accreditation standards
Support audit readiness (file completeness, consistency, documentation accuracy)
Assist with periodic reassessments and supervisory visit preparation
Active Licensed Practical Nurse (LPN) license in New Jersey
1+ year experience in home care, long-term care, or clinical setting preferred
Strong clinical observation and documentation skills
Excellent communication and interpersonal skills
Ability to work independently and manage field-based responsibilities
Reliable transportation for in-home visits
