JOB DESCRIPTION
Supervisor:
Charge Nurse
Qualifications:
Current certification as a Certified Nursing Assistant for long-term care by the New Jersey Board of Nursing. Previous experience in a geriatric long- term care facility preferred. Ability to read, write, speak and understand the English language required.
Purpose:
The primary purpose of this position is to provide the residents of the facility with routine daily nursing care in accordance with our established procedures and as may be directed by your supervisor.
Functions:
Ability to follow established performance standards and perform duties in accordance with established policies and procedures.
Ability to assist new employees in following established policies and procedures.
Ability to identify special resident problems and report them immediately to the Charge Nurse.
Ability to provide direct resident care as assigned. Ability to conduct resident rounds as assigned.
Ability to initiate corrective action as necessary and/or seek assistance from the Charge Nurse or Director of Nursing.
Ability to attend staff meetings and at least 12 hours of in-service programs per year, as assigned.
Ability to assume personal responsibility for following facility procedures related to control of equipment and supplies within the Nursing Unit
Ability to assume accountability for compliance with federal, state and local regulations within the Nursing Unit and within your span of control.
Ability to report unusual problems or incidents to the Charge Nurse or Director of Nursing.
Ability to document incidents and/or unusual problems in accordance with facility procedures.
Ability to participate in resident care conferences and other facility meetings as assigned.
Ability to participate in the development of an individualized plan of resident care as assigned.
Ability to review care plans and perform nursing care as outlined.
Ability to participate in and receive the nursing report upon reporting for duty.
Ability to document in the nursing assistant's notes the care and treatment provided to the resident and the resident's response or lack of response to care provided. Ability to assess and document weights taken, meals eaten, toileting, etc.
Ability to report changes in resident condition to the Charge Nurse or Director of Nursing.
Ability to perform basic nursing skills as assigned.
Ability to follow facility procedures for admission, discharge and transfer of residents.
Ability to inventory and mark resident's personal possessions as assigned.
Ability to listen to resident and family/sponsor complaints and report to Charge Nurse or Director of Nursing.
Ability to respond to resident calls promptly. This includes assuring the call system is within easy reach of the resident.
Ability to prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.).
Ability to perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, take to bathroom etc.)
Ability to serve meal trays, assure proper diet is served and assist and/or feed residents as necessary: (7:00 a.m.
