Job Overview
Location: 650 Bloomfield Avenue
Schedule: Full-time, 40 hours per week, 9:00 am – 5:00 pm, onsite and in the community
Hourly: $24.04

Job SummaryAs a Life Set Specialist, you will work as part of a team with young adults ages 17–22 who are transitioning into adulthood and maturing out of DCP+P care. In this community-based program, you will work with 8–10 adults in an evidence-based model to help them achieve identified goals, including housing, employment, and education.
What You Will Do- Provide individual services to 8–10 young adults in home and community settings to support transition to adulthood.
- Meet at least once per week face-to-face with the young person and provide additional support via phone, text, or in person as needed.
- Directly responsible for all interventions for a specific young adult and any necessary resources or supports.
- Assist a young person in identifying goals and work with them to achieve those goals.
- Conduct ongoing assessment of the young person’s needs from a strength-based, solution-focused perspective and develop monthly service plans.
- Document all service contacts within the timelines required by the organization.
- Be on-call rotation and respond in person when determined necessary by the Supervisor, Life Set Model Expert, and Program Director.
- Attend and actively participate in trainings and individual and team supervision sessions.
- Develop and follow up on, and/or implement feedback and recommendations from the Supervisor, Program Director, and Life Set Model Expert.
- Work will be in Bergen, Passaic, Hudson, Morris, and Essex counties.
- Bachelor’s degree in social services with one year of related experience in counseling and/or case management or related field required; master’s degree in social work, psychology, or a related field is preferred.
- Valid driver’s license and insurable under Agency automobile insurance.
- Ability to maintain a flexible schedule.
- Bilingual Spanish preferred.
