LPN needed for a Skilled Nursing Facility in Bloomfield, NM. You will provide hands on nursing care for long term care and rehab residents, including medication administration, treatments, wound care, monitoring changes in condition, and communicating updates to the RN and provider team. This is a busy SNF environment where strong time management, teamwork, and consistent documentation are key. Candidates should be comfortable supporting multiple residents, handling routine admissions and discharges, and providing compassionate care to a largely geriatric population.

Active LPN license required (NM) BLS required (facility dependent) Recent SNF or long term care experience preferred Comfortable with med passes, injections, tube feeds, wound care, vitals, and basic assessments within LPN scope Experience with EMR charting and accurate documentation required Floating within the facility may be required based on census Must be able to pass background, drug screen, and complete all facility compliance items on time Flexibility with scheduling, including weekends and holidays as needed
