Responsibilities- Provide competent nursing care to patients in on‑site clinical settings.
- Assess patient conditions and document changes accurately in charts.
- Administer medications and monitor effects per physician orders.
- Ensure compliance with regulatory standards and safety protocols.
- Collaborate with care teams to create and adjust care plans.
- Utilize performance metrics and pipelines to track personal targets.
- Maintain organized, up‑to‑date documentation for audits and reviews.
- Participate in a collaborative team environment while owning individual results.
- Proactively identify process improvements to boost efficiency and outcomes.
- Deliver patient education and health coaching to promote self‑management.
- Assist with basic triage and prioritization of patient needs on‑site.
- Support onboarding and mentoring of less‑experienced staff as needed.
- Licensed Practical Nurse (LPN) license valid for the state of employment.
- Provide direct nursing care in on‑site clinical settings with documentation.
- Accurately assess and document patient conditions and changes.
