LVN ​/ LPN – LTC​/SNF

Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN)

Seeking a licensed vocational nurse (LVN) or licensed practical nurse (LPN) for a contractual assignment in a long-term care or skilled nursing facility setting. The role involves providing compassionate patient care in a fast-paced environment to support patient wellbeing and recovery.

Responsibilities:

• Administer skilled nursing care to residents according to care plans.
• Document patient conditions and treatment updates accurately.
• Collaborate with healthcare team members to ensure optimal patient outcomes.
• Follow all facility protocols and safety procedures diligently.
• Maintain patient safety by adhering to infection control policies.
• Ensure compliance with vaccination and identification requirements.

Qualifications:

• Valid LVN or LPN license in the relevant state.
• Proven experience working in LTC or SNF environments preferred.
• Ability to bring a valid state or federal-issued  wear a photo badge at all times.
• Must have completed COVID-19 vaccination or have an approved medical/religious exemption.
• Strong communication and compassionate patient care skills.
