LVN ​/ LPN – LTC​/SNF

Long-Term Care Nursing Position

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure patient comfort, safety, and compliance with all healthcare regulations.

Responsibilities:

• Assess and monitor patient health conditions regularly.
• Administer medications and treatments as prescribed by physicians.
• Communicate effectively with residents, families, and healthcare team members.
• Document patient notes accurately and timely in medical records.
• Follow facility policies regarding infection control and safety protocols.
• Adhere to all regulatory requirements including identification and vaccination policies.

Qualifications:

• Possess a valid LVN or LPN license in the state of practice.
• Demonstrate proficient clinical skills relevant to LTC/SNF environments.
• Have completed required vaccinations or possess valid exemption documentation.
• Exhibit strong communication and interpersonal abilities.
• Maintain flexibility to work a 7-week contract with a 36-day schedule.
• Ability to wear a photo badge and present state or federal-issued .
