LVN ​/ LPN – LTC​/SNF

Long-Term Care Nursing Position

Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting. Ensure the delivery of high-quality patient care through effective assessment and intervention.

Responsibilities:

• Assess patient conditions and develop tailored nursing care plans.
• Administer medications and treatments following regulatory guidelines.
• Monitor patient vital signs and report changes to the healthcare team.
• Document patient care accurately and maintain compliance with facility policies.
• Educate patients and their families on health management and post-discharge care.
• Adhere to infection control protocols and safety procedures at all times.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Successful completion of COVID-19 vaccination or documented exemption.
• Previous experience in LTC/SNF settings preferred.
• Ability to assess patient needs and respond appropriately.
• Strong communication and interpersonal skills.
• Capable of working effectively in a fast-paced environment.
