LVN ​/ LPN – LTC​/SNF

Nursing Care Provider

Provide compassionate nursing care to residents in long-term care or skilled nursing facilities. Ensure residents’ health and safety through effective clinical practices and collaboration with interdisciplinary teams.

Responsibilities:

• Administer prescribed medications and treatments accurately and timely.
• Monitor and assess residents’ health status, reporting any changes to the healthcare team.
• Document all patient interactions and clinical interventions according to facility policies.
• Follow infection control protocols, including COVID-19 vaccination and safety guidelines.
• Maintain a clean and safe environment, adhering to regulatory standards.
• Wear identification badges and comply with facility access requirements.

Qualifications:

• Valid and current LVN or LPN license in the state.
• Recent experience in LTC/SNF settings preferred.
• Ability to demonstrate understanding of elder care nursing procedures.
• Must have completed or be willing to obtain COVID-19 vaccination or an approved exemption.
• Strong communication and interpersonal skills.
• Ability to work effectively in a team environment.
