LVN​/LPN – LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting as an LVN/LPN. Ensure the delivery of compassionate, patient-centered services in accordance with established protocols.

Responsibilities:

• Administer medications and treatments following detailed care plans.
• Monitor residents’ health status and report changes to supervising clinicians.
• Assist with daily living activities and ensure resident comfort.
• Maintain accurate documentation of care provided and resident responses.
• Follow infection control procedures and promote a safe environment.

Qualifications:

• Valid LVN/LPN license in the state.
• Proven experience in LTC or SNF environments preferred.
• Ability to work efficiently in a fast-paced setting.
• Strong communication and interpersonal skills.
• Knowledge of federally mandated vaccination requirements.
• Capable of adhering to strict identification and signage protocols.
