LVN​/LPN – LTC​/SNF

Job Summary:

Provides basic nursing care and support to residents in a long-term care or skilled nursing facility setting. Assists with daily patient needs, monitors health status, and ensures compliance with healthcare protocols.

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers.
• Document patient observations, care provided, and any changes in condition.
• Assist residents with activities of daily living, including bathing, dressing, and mobility.
• Follow infection control procedures and maintain a safe environment.
• Communicate effectively with patients, families, and the healthcare team.
• Adhere to facility policies, including identification requirements and COVID-19 vaccination protocols.

Qualifications:

• Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) credential.
• Valid state license in good standing.
• Proven experience in long-term care or skilled nursing environments preferred.
• Ability to follow detailed instructions and cooperate with healthcare teams.
• Strong communication and interpersonal skills.
• Commitment to patient safety and quality care.
