LVN​/LPN – LTC​/SNF

Job Summary:

Provide Provide compassionate and high-quality nursing care to residents in a long-term care or skilled nursing facility setting. Ensure residents' health, safety, and comfort through clinical expertise and attentive monitoring.

Responsibilities:

• Administer nursing care and treatments according to physicians' orders and facility policies.
• Monitor residents' vital signs and respond promptly to changes in condition.
• Document resident assessments and care activities accurately and thoroughly.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Ensure compliance with all state and federal regulations, including COVID-19 vaccination requirements.
• Maintain a safe, clean, and organized environment, and wear required identification and badges.

Qualifications:

• Valid and current LVN or LPN license in the state of practice.
• Minimum of 1 year of experience in LTC/SNF or similar long-term care environment.
• Ability to follow detailed policies regarding COVID-19 vaccination or exemption documentation.
• Strong clinical skills with attention to residents' safety and well-being.
• Excellent communication and interpersonal skills.
