LVN​/LPN – LTC​/SNF

Job Summary:

Seeking a licensed practical nurse (LPN) to provide skilled nursing care in a long-term care facility. The LPN will support patient needs, implement care plans, and collaborate with the healthcare team to ensure optimal patient outcomes.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Document patient care activities promptly and precisely.
• Monitor patient vital signs and respond to changes in condition.
• Collaborate with registered nurses and physicians to coordinate patient care.
• Educate patients and their families about health management and treatments.
• Adhere to facility policies and safety procedures at all times.

Qualifications:

• Valid LPN license issued by the state of New Mexico.
• At least 1 year of experience in LTC/SNF settings preferred.
• CPR certification required; additional clinical certifications are a plus.
• Demonstrate strong communication and teamwork skills.
• Ability to handle multiple patient care responsibilities efficiently.
