Job Summary:
Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a contract position in a skilled nursing facility. The role involves providing high-quality patient care in a fast-paced LTC/SNF environment, ensuring compliance with healthcare standards.
Responsibilities:
- Administer medication and deliver essential patient care under the supervision of registered nurses and physicians
- Monitor patient conditions and document observations accurately in medical records
- Assist with patient mobility, personal hygiene, and nutritional needs
- Adhere to clinical protocols and safety procedures to maintain a sterile environment
- Communicate effectively with patients, families, and healthcare team members
- Comply with facility policies regarding identification, badges, and vaccination requirements
Qualifications:
- Valid LVN or LPN licensure in the relevant state
- Proven experience in long-term care or skilled nursing facilities
- Ability to follow detailed instructions and maintain high standards of patient care
- Knowledge of medication administration and documentation procedures
- Must have completed the COVID-19 vaccination or hold an approved exemption
- Strong interpersonal and communication skills
