LVN​/LPN – LTC​/SNF

$1217.88/week –

Job Summary:

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting. Ensure the delivery of safe, effective, and compassionate care in accordance with established protocols and regulations.

Responsibilities:

• Administer prescribed medications and treatments accurately and timely.
• Monitor and document residents’ health status and progress.
• Assist with activities of daily living to promote dignity and independence.
• Collaborate with multidisciplinary team to develop and update care plans.
• Maintain compliance with facility policies, procedures, and safety standards.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience in LTC or SNF settings preferred.
• Competency in administering medications and basic nursing procedures.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
