Residential Home Health and Hospice is looking to add a home health occupational therapist to our interdisciplinary team. This Light full‑time position supports patients in Schaumburg, IL and surrounding areas and is eligible for a $8,000 first‑year success bonus to reward key achievements during the first year. The therapist will collaborate to manage and deliver patient care plans.

• Collaborate with the interdisciplinary team to manage and deliver patient care plans.
• Support patients in Schaumburg, IL and surrounding areas.
• Self‑manage schedule and meet critical charting deadlines.

• Bachelor of Science or Master’s degree in Occupational Therapy from a school accredited by the Council of Medical Education of the American Medical Association and the American Occupational Therapy Association.
• Licensed Occupational Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Ability to self‑manage time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.
