Job Overview

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health for a nursing job in Bloomingdale, Illinois. The position is full‑time (40 hours per week) and supports patients in Schaumburg, Arlington Heights, Rolling Meadows and surrounding areas.

Home health registered nurse case managers lead and collaborate to provide management and delivery of patient care plans. Duties include patient assessments, care plan development, coordination with interdisciplinary teams, charting, driving to patient locations and ensuring timely completion of documentation.

• RN license in the state where you will work.
• Graduate of an approved school of practical nursing.
• One year of RN experience with clinical competence to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Self‑management of time and schedule.
• Demonstrated experience with tablets, mobile phones and EMR software.
