Signify Health in Illinois is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. This part-time role involves 12 hours per week with a base pay range of $46.03–$99.14 per hour, including incentives where applicable.
Clinicians will assess health history, perform physical evaluations, and may conduct additional tests as needed. Qualified candidates hold an active license, board certification, and preferably multi-state licensure.
