In-Home Health NP​/PA Clinician — Transforming Care

Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with people in the comfort of their own homes and bridging gaps in care.

You will travel locally and within licensed states, conduct virtual visits when needed, and perform diagnostic and preventive services using an iPad and clinical workflow. A strong license and board certification are required.
