We are seeking a highly motivated, qualified Illinois licensed nurse to join our team as a Nurse Practitioner. The Nurse Practitioner is responsible for coordinating patient care to ensure high-quality, cost-effective, and patient-centered outcomes. This role involves assessment, care planning, utilization management, patient advocacy, and collaboration to support optimal health outcomes.

Qualifications
  • Current certification as a Nurse Practitioner (NP) from an accredited program
  • Current and valid RN license for the state of Illinois
  • Master's degree in Nursing
  • Must have at least two years of nursing experience, preferably in skilled nursing, long-term care, rehabilitation, or post-acute care
  • Previous experience in case management, discharge planning, or care coordination in a SNF or post-acute setting preferred
  • Case Management Certification (CCM, ACM, or equivalent) preferred or willingness to obtain
  • Knowledge of Medicare Part A, managed care, and skilled-nursing facility reimbursement processes
  • Excellent clinical assessment and problem-solving skills
  • Strong communication skills with residents, families, providers, and payer representatives
  • Must be charismatic, energetic, and people-oriented
  • Good working knowledge of computers
