Overview

The LPN assigned to PACT/Medical Specialty clinics is accountable for delivering direct patient care to adult through geriatric Veterans for both acute and chronic diseases in an outpatient clinic setting. The LPN recognizes age-related needs and demonstrates knowledge and care appropriate to those age groups, considering the cognitive, physical, emotional, and chronological maturation needs of patients.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements
  • Education:

    Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by ACEN (formerly NLNAC) at the time the program was completed
  • Health care education obtained in the military service or training in the military service which was accepted by the licensing body in the jurisdiction in which you are licensed
  • Granted a license by a jurisdiction that does not require graduation from an approved school AND have at least one year of successful practice as an LPN/LVN
  • Licensure:
    Must possess a Full-active-current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (e.g., Puerto Rico) or District of Columbia

Grade Determinations
  • GS-3 Level: None beyond the basic requirements
  • GS-4 Level: Must have completed six (6) months of qualifying experience as an LPN/LVN or graduation from an approved school with at least one year of nursing care experience in hospital, outpatient clinic, nursing home, or similar setting; or graduation from an approved school of at least 24 months duration (transcripts required)
  • GS-5 Level: Must have at least 1 year of qualifying experience at the GS-4 level or equivalent; able to provide a full range of practical nursing care to patients with various physical or behavioral problems
  • GS-6 Level: Must have at least one year of additional qualifying experience at the GS-5 level or equivalent; proficient in initiating, performing, and completing assigned duties in patient care; able to organize and deliver care considering patient conditions
Responsibilities and Practice
  • Work with RN and/or MD/DO to orient/train less experienced LPNs/LVNs and Nursing Assistants or Health Technicians in acceptable standards of practice
  • Provide education to patients and/or families regarding common disease processes, medications, and prescribed treatment regimes
  • Prepare, administer, and document actions for prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) as permitted by local policies; observe, document, and report patient responses
  • Recognize emergencies and initiate appropriate emergency interventions as directed
  • Identify and report equipment malfunctions or safety hazards promptly
  • Maintain general supervision under RN or MD/DO; demonstrate broad working knowledge of practical nursing procedures
  • Assess and respond to patient needs for care, including medication administration, equipment-assisted care, and patient/family education
  • Prepare patients for diagnostic tests and assist with procedures; monitor patient condition before, during, and after procedures
  • Provide patient education and contribute to planning, implementing, documenting, and evaluating care in outpatient settings
  • Respect lifestyle values, beliefs, and practices of patients; incorporate customer service standards in all interactions
  • Demonstrate ability to work with diverse patient populations and provide care in accordance with established policies

    Monday - Friday, 7:45 AM – 4:15 PM

    Competitive salary with regular increases; higher steps may be considered based on qualifications
