- Schedule:
8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most.
Your day-to-day includes:
- Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
- Building and adjusting personalized care plans in collaboration with physicians and care teams
- Administering medications and treatments per physician orders
- Keeping clinical documentation accurate, timely, and meaningful
- Serving as the communication bridge between families, physicians, and community health partners
- Educating patients and caregivers so they feel confident and supported
- Participating in on-call rotation per team policy

- Active RN license in the state of employment
- Graduate of an accredited school of Nursing.
- One (1) + year Hospice, home health or related organization experience preferred.
- Compassion for patients and families navigating end-of-life care
- Strong clinical judgment and the ability to work independently in the field
- Excellent communication skills across care teams and families
