Wellness Nurse

Bickford Senior Living is seeking a compassionate Wellness Nurse to support residents in our assisted living and memory care community. In this resident-first, family-oriented setting, you'll provide daily nursing care, medication management, and health assessments while fostering dignity, independence, and safety. Partner with caregivers, families, and physicians to develop and follow individualized care plans. You'll build meaningful relationships, respond to changes in condition, and support a warm, home-like environment.

Responsibilities

• Provide daily nursing care, including assessments, treatments, and monitoring of residents' health status
• Administer medications safely and accurately according to care plans and regulations
• Document care, changes in condition, and interventions in EHR or charts
• Collaborate with caregivers, physicians, and families to support resident-centered care plans
• Support residents with ADLs while promoting dignity, independence, and safety
• Respond to emergencies and follow appropriate clinical and safety protocols
• Participate in care conferences, training, and quality improvement initiatives
• Educate residents and families on health, wellness, and care plans
