Licensed Practical Nurse

142013 Westminster Village Inc is seeking a compassionate Licensed Practical Nurse to provide direct care to residents in our continuing care retirement community. The LPN will administer medications and treatments, monitor and document resident conditions, assist with ADLs, and support physicians and RNs in coordinating care across assisted living, memory care, rehab, and skilled nursing. This role promotes person centered, dignified care while collaborating with a close knit interdisciplinary team.

Responsibilities

• Provide direct nursing care to residents across assisted living, memory care, rehab, and skilled nursing levels
• Administer medications and treatments in accordance with provider orders and facility policies
• Monitor, document, and report changes in resident condition to RNs and physicians
• Assist residents with activities of daily living while promoting dignity and independence
• Support care planning and interdisciplinary team coordination for resident care
• Maintain accurate, timely clinical documentation in electronic health records
• Follow infection control, safety, and regulatory standards at all times
• Communicate effectively with residents, families, and team members to support person centered care
