School Case Manager – Devil's Lake, ND School District
About the Role

Join our Nexus‑PATH team as we support our school case management services to the Devil's Lake, ND area schools.
Job TypeFull‑time (12‑month assignment; primarily on‑site at local school)
Pay & LocationSalary range: $48,000–$54,000. On‑site at local school.

- Provide comprehensive coordination of all tasks required to meet the treatment needs and best interests of service recipients consistent with regulatory agencies (i.e., DHS, COA)
- Comply with local office procedure for referral and service delivery
- Complete a systemic assessment of the needs of the child and family
- Establish service parameters with family or legal custodian as child’s needs dictate
- Provide complete and required documentation, within established timelines, relating to case management and treatment planning
- Implement treatment services to meet the needs and goals of the children and families utilizing quality case management practice to provide for the best interest of the child and family
- Provide direction to other team members when applicable and make recommendations for discharge, finalization and permanency
- Promote the needs of youth and families to ensure successful outcomes
- Advocate for the development and maintenance of funding and resources to meet youth and family needs
- Develop and maintain community contacts to promote Nexus‑PATH and collaborative efforts
- Provide crisis intervention as needed to maintain safety and provide for the best interest of the child and family
- Integrate trauma‑informed care principles into practice by sharing knowledge and understanding of trauma with the child, family and relevant team members
- Utilize and advocate for the use of trauma‑sensitive strategies to support the youth and their family
- Routinely screen for trauma exposure and related symptoms and demonstrate competence in using screening tool results to enhance service provision
- Routinely make resources available and share knowledge with foster families and providers on trauma exposure, its impact, and trauma‑focused treatment
- Engage in efforts to strengthen the resilience and protective factors of children and families impacted by and vulnerable to trauma
- Emphasize continuity of care and collaboration across child‑service systems
- Understand potential for secondary traumatic stress and practice self‑care strategies to mitigate these effects
- Meet standards for productivity and caseload size while maintaining appropriate documentation and files to support the services that are provided to families
- Participate in staff and supervision meetings and required Nexus‑PATH in‑service meetings
- Recognize and value cultural differences in all aspects of work and service delivery
- Ensure confidentiality of all treatment and services information
- Bachelor’s degree in social work preferred; will consider bachelor’s degree in a human services related field
- 1–3 years of work experience serving children and families
- Current North Dakota social work licensure preferred
- Experience in family‑based service strongly preferred
- Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards
- Must successfully pass a DHS/DCFS/CJIS/DHHS/ODHS or other federal and state criminal background check
- Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges
- Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values
