POSITION TITLE:

CLASSIFICATION:
Non-Exempt

PURPOSE:

Integrate, coordinate, and monitor the active treatment program and medical services for each client receiving services. Represents the client to ensure that the individual receives needed services and interventions from providers who deliver quality services. Maintain person-centered plans. Provide supervision to the Team Managers to achieve the goal of ensuring that the client receives necessary services and interventions. Ensure caregivers are trained and capable of delivering the needed services to clients.

KEY AREAS OF RESPONSIBILITY:

1. Write training plans, and case conference agreements for clients in the Supported Living program. Following Life Course framework model.

2. Develop, write, and implement training plans for clients regarding medical issues, including but not limited to BQIS regulations regarding medications and side effects, seizure plans and precautions, dehydration and constipation, nutritional/dietary needs, and safety/behavioral needs.

3. Attend and participate in quarterly and annual case conferences and other meetings related to clients.

4. Complete and maintain all required paperwork for clients.

5. Review monthly data documentation and write monthly progress reports for clients. Upload to BDDS portal as required monthly.

6. Ensure documentation of Supported Living services is completed and accurate, including but not limited to daily notes, MARS, training plans, risk plans, and any other necessary documentation.

7. Coordinate the delivery of approved services between selected service providers. Serve as a liaison between clients, families, staff, external professionals, and the community.

8. Coordinate and evaluate the instructional activities that are provided to clients.

9. Complete incident reports, including BDDS, and needed follow-ups within 24 hours of incident notification.

10. Participate in monthly Case Record Review committee meetings and record review follow-up.

11. Supervise Team Managers, provide client-specific trainings, and meet weekly. Evaluate the performance of Team Managers at 90 days and annually. Assist Team Managers with reviews of DSPs at 90 days and annually, as needed.

12. Audit and approve the time of Team Managers. Assist Team Managers in auditing and approving DSP time, as needed. Audit and approve mileage for all Supported Living staff.

13. Ensure services are provided in compliance with all funding sources and governing bodies (CARF, BDDS, BQIS, State and Federal Law).

14. Meet monthly with the Regional Director and Senior Director, Residential Services to discuss personnel related issues, client programming and compliance/incident reporting.

15. Assist the Director of Community Services, as needed.

16. Assist with the review of billing and time entry for accuracy and compliance with regulations.

17. Provide direct care as needed and participate in the on-call rotation.

18. Display patience in all situations involving clients. Speak in an even, positive, and professional tone of voice when conversing with clients.

19. Appropriately respond to client's behavior via verbal and/or physical intervention based on the needs of the client.

20. Be readily available to answer staff questions and/or schedule time to meet with Residential staff and route them to the appropriate department, if needed.

21. Plan, schedule, and conduct divisional orientation with new Residential Staff.

22. Strictly maintain confidentiality of all clients and staff.

23. Ensure that employees follow all departmental and agency policies, as well as state and federal laws. Conduct review of policies.

24. Review the hours planned and scheduled as needed to ensure client budgets are being utilized in accordance with their allocations with the online software, Accel and Provide.

25. Maintain working knowledge of company-wide system. Lead and attend trainings as needed with the IT department.
