Position: Care Coach
Department: Indiana Family Caregivers Care Management
Reports To: Care Coach Supervisor
Status: Part-time
Hours: Flexible to meet the needs of the Caregiver
Position Summary
The Care Coach position has been created to lend support to caregivers that are caring for a family member with complicated needs. As a care coach, you will go to the homes of our caregivers and help them find resources, offer support, and build good will. The goal of a Care Coach is to make 1 visit per quarter and build support for the caregiver.
Encouraging self-care and helping the caregivers set goals for themselves. This position will support Cluster J (Hancock, Shelby, Rush, Fayette, Union, Franklin, Decatur, Monroe, Brown, Bartholomew, Lawrence, Jackson, Jennings, Ripley, Dearborn, Jefferson, Switzerland and Ohio counties).
Responsibilities/Job Duties
- Make home visitsas required per state guidelines to ensure compliance.
- Maintain records of visits
- Communicate with the Care Coach Supervisor
- Travel within the assigned territory to seecaregiversin their homes.
- Coordinate visit times with the caregiver to ensure they are scheduled at mutually convenient times and commit to honoring scheduled appointments—cancelling only in true emergencies.
- Document notes fromvisitinrequired apps on computer/ phone.
- Attend monthly team meetings (virtually).
- Perform other duties as assigned.
Required Qualifications
- High school diploma or equivalent
- Strong understanding of the emotional and physical demands placed on caregivers.
- Experience working with medically complex populations.
- Must be detail oriented.
- Demonstrate good organizational skills and time management skills.
- Must have the ability to travel on a weekly basis as this role is a field-based position.
- Must have an active driver license and reliable transportation with insurance to drive throughout the day / territory.
- Ability to effectively communicate with patientsandfamilies.
- Strong critical thinking and problem-solving skills.
- Musthave your own computer/ iPad or tablet and cell phone.
