• Assess patients’ psychosocial, financial, and environmental needs
• Complete initial and ongoing social work evaluations
• Support care planning and discharge planning
• Connect patients and families with community resources
• Communicate changes and concerns with care team and physicians
• Document visits and maintain compliance with care standards
• Provide counseling and guidance to patients and caregivers

• Master’s or Doctoral Degree in Social Work (CSWE accredited)
• Active Social Worker license
• 1+ year of healthcare social work experience
• Strong communication and interpersonal skills
• Ability to travel within the coverage area (50%)
• Valid driver’s license, auto insurance, and reliable transportation
• Ability to lift, push, or pull up to 50–100 lbs
