What You'll Do

* Provide skilled nursing care in accordance with physician orders and patient care plans

* Review and follow the Plan of Care during each patient visit

* Monitor patient conditions and communicate changes with the RN Case Manager and care team

* Educate patients and caregivers on disease management and self-care techniques

* Document patient care accurately and timely

* Collaborate with physicians, clinicians, and interdisciplinary care teams

* Support efforts to reduce avoidable hospitalizations and improve patient outcomes

* Deliver compassionate care and exceptional patient experience

What You'll Need

* Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

* Current Indiana LPN license

* Minimum of 1 year of nursing experience in a clinical care setting

* Home Health or Hospice experience preferred

* Reliable transportation, valid driver's license, and current auto insurance

* Ability and willingness to travel within assigned territory

* Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
