As a Registered Nurse (RN), you will provide one‑on‑one patient care, working at the top of your license to give patients the greatest gift – the ability to spend enhanced quality time with loved ones in their preferred environment. You will utilize your clinical skills in providing end‑of‑life care as directed by an attending physician and be part of an interdisciplinary team focused on compassionate quality care.

Responsibilities- Oversee overall patient care from beginning to end of life, observing and monitoring to ensure care meets patient needs; work autonomously within the scope of practice.
- Plan of Care: evaluate treatment and medical condition per the plan, individualize and adjust as needed.
- Minimize Patient Risk: update all doctor orders in the patient chart and administer medication as prescribed.
- Continuity of Care: manage multi‑disciplinary care, promote continuity with admissions, transfers, and discharges.
- Family Educator/Advocate: counsel patient and family on disease progression, incorporate them into plan development, and listen to their concerns.
- Policies: complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Operations: oversee and supervise patient care provided by nurse aides and LPNs; attend in‑service training and mandatory agency meetings.
- Compassionate communicator with a positive attitude.
- Patience when working with patients, families, physicians, and coworkers.
- Attention to detail, observant, and follows directions.
- Registered Nurse with current license in the state of employment.
- Minimum one‑year experience as an RN in an acute care setting preferred; hospice experience desired.
- Valid driver’s license and auto insurance in your name as a driver.
- Capable of all physical demands.
