The Licensed Practical Nurse (LPN) Assisted Living
-Weekend Option plays a critical role in delivering compassionate and professional nursing care to residents in a supportive living environment. This position focuses on ensuring the health, safety, and well-being of residents by administering medications, monitoring vital signs, and assisting with daily living activities. The LPN collaborates closely with healthcare teams, families, and residents to develop and implement individualized care plans that promote independence and quality of life.

Additionally, the role involves maintaining accurate medical records and adhering to regulatory standards to ensure compliance and high-quality care delivery. Ultimately, the LPN contributes to creating a warm, respectful, and therapeutic environment that supports the physical and emotional needs of each resident.

Job Responsibilities

  • Administer prescribed medications and treatments accurately and timely according to established protocols.
  • Monitor and document residents' health status, including vital signs and any changes in condition, and report findings to the supervising nurse or physician.
  • Assist residents with activities of daily living such as bathing, dressing, mobility, and meal assistance as needed.
  • Collaborate with interdisciplinary teams to develop, update, and implement individualized care plans tailored to each resident's needs.
  • Maintain detailed and confidential medical records in compliance with healthcare regulations and facility policies.
  • Provide emotional support and education to residents and their families regarding health conditions and care procedures.
  • Ensure a safe and clean environment by following infection control and safety protocols.

Minimum Qualifications

  • Current and valid Licensed Practical Nurse (LPN) license in the state of Indiana.
  • Basic Life Support (BLS) certification.
  • Demonstrated knowledge of medication administration and nursing care procedures.
  • Ability to communicate effectively with residents, families, and healthcare team members.

Preferred Qualifications

  • Experience working in assisted living or long-term care settings.
  • Familiarity with electronic health record (EHR) systems.
  • Strong interpersonal skills and a compassionate approach to resident care.
