As an RN Visit Nurse
, you'll provide 1:1 skilled nursing care that allows patients to recover safely, remain independent, and stay where they want to be most: at home.

You'll help patients recover after hospitalization, manage chronic illnesses, prevent complications, and remain safely at home while providing compassionate, individualized nursing care.
Instead of feeling like just another clinician, you'll become an important part of each patient's healthcare journey.
What You'll Be Doing- Provide skilled home health nursing visits throughout the St. Paul service area
- Perform comprehensive patient assessments
- Deliver wound care
, medication management, injections, and other skilled nursing services - Educate patients and caregivers on diagnoses, medications, and treatment plans
- Monitor patient progress and communicate changes with physicians
- Collaborate with therapists, social workers, and interdisciplinary care teams
- Complete timely documentation in Homecare Homebase (HCHB)
- Promote positive patient outcomes while helping reduce unnecessary hospitalizations
- Active Minnesota Registered Nurse (RN) license
- Experience in home health
, hospital, rehabilitation, skilled nursing, or other clinical settings - Strong assessment and critical thinking skills
- Excellent communication and relationship-building abilities
- Ability to work independently while managing daily visits
- Reliable transportation and willingness to travel throughout the St. Paul territory
