Monday through Friday from 4:00 PM to 8:30 AM, with every other weekend required.
This position follows a 7 days on / 7 days off rotation, providing a balanced work schedule with a full week off between rotations.
As a Registered Nurse (RN) you will provide one‑on‑one patient care and support a team of nurses and patients by responding to after‑hours patient needs. You will be part of an interdisciplinary team that focuses on delivering compassionate quality care and positive outcomes. Your role includes interacting with patients' families, providing education, and contributing to every step of their recovery journey.

How you'll workAssume the primary role of skilled nursing care in patient homes as directed by the attending physician. Work autonomously, managing your time efficiently. Be responsible for all duties within the scope of practice defined by the state. When after‑hours needs are minimal, assist with chart review or patient phone calls from your home.
Major areas of responsibility- Plan of Care: observe and monitor patient conditions, perform OASIS assessments, develop individualized care plans, and adjust as needs change.
- Minimize Patient Risk: administer medications as prescribed and prioritize visits for high‑risk patients to reduce readmissions.
- Supervision: oversee nursing care provided by aides and LPNs.
- Continuity of Care: manage multidisciplinary care, promote continuity through admissions, transfers, and discharges.
- Family Educator/Advocate: educate patients and families on disease processes using teach‑back methods to ensure understanding.
- Policies: complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Collaborate with interdisciplinary team and physicians to ensure optimal care.
- Rules and Regulations: follow agency policies, communicate schedule changes to schedulers or supervisors.
- Operations: attend in‑service training and mandatory agency meetings.
- Chart Review: complete chart reviews as needed to support patient care and regulatory compliance.
- Compassionate communicator with a positive attitude.
- Patience when working with patients, families, physicians, and coworkers.
- Attention to detail and observant of directions.
- Registered Nurse with a current license in the state of employment.
- Minimum one‑year experience as an RN in an acute care setting.
- Home care experience preferred.
- Valid driver’s license and auto insurance in your name as a driver.
- Designated workspace at home for chart reviews.
- Capable of meeting all physical demands of the position.
