About the Role:The Case Facilitator I
- Adult plays a critical role in supporting adults through the coordination and facilitation of case management services. This position is responsible for ensuring that clients receive timely and appropriate assistance by connecting them with necessary resources and services. The role involves assessing client needs, developing individualized plans, and monitoring progress to promote positive outcomes. The Case Facilitator acts as a liaison between clients, service providers, and community organizations to streamline communication and service delivery.
Ultimately, this position aims to empower adults to achieve greater self-sufficiency and improved quality of life through effective case facilitation.
- High school diploma or equivalent.
- Experience working with adult populations in a social service or case management setting.
- Basic knowledge of community resources and social service programs.
- Strong organizational and communication skills.
- Ability to maintain confidentiality and handle sensitive information with professionalism.
- Experience with case management software and electronic record keeping.
- Familiarity with mental health, substance abuse, or housing services.
- Training in crisis intervention or conflict resolution.
- Bilingual abilities or experience working with diverse populations.
- Teach and model activities of daily living skills.
- Develop and implement individualized case plans in collaboration with clients and relevant stakeholders.
- Coordinate referrals to community resources, healthcare providers, and social services.
- Monitor client progress and update case records to ensure accurate documentation and follow-up.
- Facilitate communication between clients, service providers, and internal teams to resolve issues and improve service delivery.
- Provide ongoing support and advocacy to clients to help them navigate complex systems and overcome barriers.
- Participate in team meetings and training sessions to stay informed about best practices and available resources.
The required skills such as strong communication and organizational abilities are essential for effectively coordinating services and maintaining accurate case documentation. The ability to assess client needs and develop individualized plans ensures that each client receives tailored support. Education:
High school diploma or equivalent.
Experience:
Must meet OBHP requirements. Two years’ experience in mental health or community services preferred.
Licenses:
Must possess a valid state issued Driver’s License.
Shift: First shift, full time. Monda-Friday, 8am to 5pm with some evening hours to reach all clients.
Salary based on experience.
