The Case Manager provides structured, goal-oriented coordination of services for pregnant and postpartum mothers and families. This role is responsible for assessing needs, developing individualized service plans, coordinating referrals, and ensuring families are connected to appropriate medical, behavioral health, housing, and social support services.
The Case Manager serves as the primary system navigator and care coordinator, ensuring that all aspects of a family’s service plan are organized, documented, and progressing toward measurable outcomes.
Key Responsibilities Assessment & Case Planning- Conduct intake assessments to identify family strengths, needs, and risks
- Develop individualized service plans with clear goals and timelines
- Regularly review and update case plans based on progress and changing needs
- Connect families to community resources including:
- Healthcare and prenatal/postpartum care
- Mental and behavioral health services
- Housing and financial assistance programs
- Early childhood and developmental services
- Coordinate services across multiple agencies and providers
- Track participation and progress toward case plan goals
- Ensure required documentation is completed accurately and on time
- Monitor service engagement and follow-up on missed appointments or gaps in care
- Communicate with healthcare providers, social service agencies, and program partners
- Participate in multidisciplinary team meetings to review family progress
- Support compliance with program, grant, or regulatory requirements
- Maintain detailed case notes, service plans, and outcome tracking
- Prepare reports for program evaluation and funding requirements
- Ensure confidentiality and proper record-keeping standards
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred
- Experience in case management, social services, or care coordination required
- Strong organizational, documentation, and systems-navigation skills
- Knowledge of community resources and service networks preferred
- Ability to manage multiple cases and deadlines effectively
