The Dietitian is primarily responsible for providing service in accordance with a plan of treatment and care plan for the Home Health Agency.
Service Area: Twin Cities Metro area. Coverage areas include:
St. Louis Park, Richfield, Bloomington, Burnsville, Lakeville, Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul Park, Rosemount, Eagan, Woodbury, Roseville, Arden Hills, New Brighton, Shoreview, Mounds View, Circle Pines, Lino Lakes, Forest Lake, Wyoming, Scandia, Hugo, White Bear Lake, Mahtomedi, Stillwater, Lake Elmo, Woodbury, Oakdale, North St. Paul and surrounding.
Plus mileage reimbursement of 72.5 cents per mile.
Schedule: Casual/Part-Time

Position Functions & Responsibilities:
- Observes and evaluates treatment effect and client’s overt physical/emotional response to treatment goals. Updates treatment plan based on evaluation and as new orders are received.
- Assesses the rehabilitative status of the client and develops appropriate treatment plan for each client; updates and modifies treatment plan on an ongoing basis.
- Lead by example in establishing rapport with clients.
- Ensure positive & professional communication on team.
- Plans goals and care communicates with IDT.
- Observes, records and reports information on the client’s condition to the attending physician and in the client’s medical record.
- Documents client treatment and response to treatment accurately, using established guidelines.
- Honors client confidentiality, rights, privacy, and reporting maltreatment expectations.
- Follow all Emergency and Response Procedures
- Identifies home safety issues and notifies RN Case Manager or Clinical Manager of concerns or issues
- Demonstrates proper procedures: handling, lifting, transfers
- Injury – Now What? Follows protocols
- Overview of Process, Types of Reports and Forms
- Other duties as assigned
Qualifications:
- Experience working with people in the second half of life.
- Current and active Dietitian license in the state of Minnesota.
