Lifespark is seeking a compassionate Physical Therapist for Start of Care in home health in the Twin Cities Metro area. The role involves performing initial assessments, planning and directing PT services in patients' homes, and coordinating with families and the care team.
The PT will educate clients and caregivers, document progress, and ensure safety and proper use of equipment. Schedule is Monday–Friday, 8:00am–5:00pm, with no on-call or weekend duties; mileage is reimbursed.
