Center Well Home Health is seeking a licensed Physical Therapist to provide in-home rehabilitation services. You will design and implement PT plans, assess patient function, and collaborate with physicians to achieve optimal recovery.
Ideal candidates have a PT degree, an active license, CPR certification, and strong organizational and communication skills to coordinate care across the clinical team. This role is full-time in Minnesota with field-based responsibilities.
