Part Time - Nurse Practitioner or Physician Assistant

Role Overview: This position involves providing in-home health evaluations to patients, focusing on building trusted relationships and improving health outcomes. The clinician conducts comprehensive assessments, including medication and medical history reviews, physical evaluations, and optional diagnostic tests, using an iPad and clinical workflow. They do not prescribe medication or alter treatments.

• Deliver personalized health evaluations in patients’ homes or virtually
• Engage with patients to address health questions and needs
• Perform diagnostic and preventive services such as spirometry and screenings
• Travel within licensed areas as needed, ensuring safety and comfort in diverse home environments
• Build connections to appropriate care based on patient assessments

• Active, unrestricted licensing (NP or PA) and board certification
• Preferred multi-state licenses and ability to cover specific locations
• Strong interpersonal skills, compassionate approach, and adaptability
