Job Overview: The Home and Community Based Services Specialist plays a vital role in assisting individuals to maintain their independence and enhance their quality of life by providing a variety of supportive services in their homes and communities. The specialist collaborates with clients, families, and other service providers to develop and implement personalized care plans.

Client Support:

• Provide direct support to clients, assisting with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as needed.
• Offer companionship and engage clients in social and recreational activities to promote overall well-being.

Documentation:

• Maintain accurate and up-to-date records of client assessments, care plans, and service delivery.
• Document progress, outcomes, and any changes in clients' conditions or needs.

Assessment and Planning:

• Conduct comprehensive assessments of clients' needs, preferences, and goals for living independently.
• Collaborate with clients and their families to develop individualized care plans that outline the necessary services and support.
