Position Overview

We are seeking a hands‑on Director of Rehabilitation to lead therapy services within a skilled nursing facility in Willmar, Minnesota. This full‑time leadership opportunity combines direct patient care with team supervision, department operations, program development, and quality oversight.

We welcome applications from Physical Therapists (PT), Physical Therapist Assistants (PTA), Occupational Therapists (OT), and Occupational Therapy Assistants (OTA) who are interested in leading and developing a multidisciplinary rehabilitation team.

Compensation
  • $30-$48 per hour, depending on discipline and experience
  • PT and OT: $40-$48 per hour
  • PTA and OTA: $30-$35 per hour
  • Eligible for a sign‑on bonus, student loan assistance, or a combination of both
Key Responsibilities
  • Oversee the daily operations of the rehabilitation department within a skilled nursing facility.
  • Provide direct patient care in accordance with professional licensure and scope of practice.
  • Supervise, mentor, train, and evaluate therapy staff.
  • Maintain appropriate staffing levels and coordinate therapist and patient schedules.
  • Manage departmental productivity, performance, and budget expectations.
  • Complete quality assurance reports, documentation audits, and compliance reviews.
  • Develop and implement corrective action plans when needed.
  • Identify opportunities to expand rehabilitation services and strengthen therapy programs.
  • Lead staff meetings, in‑services, and educational programs.
  • Collaborate with facility leadership, healthcare professionals, patients, families, and caregivers.
  • Assist with authorizations, insurance claims, denials, and reimbursement matters as needed.
  • Maintain accurate and timely clinical and operational documentation.
  • Promote a positive, patient‑centered environment focused on high‑quality care.
Qualifications
  • Graduate of an accredited Physical Therapy, Physical Therapist Assistant, Occupational Therapy, or Occupational Therapy Assistant program.
  • Current Minnesota licensure or eligibility for licensure in the applicable discipline.
  • Five years of clinical therapy experience preferred, including previous director or management experience.
  • Experience managing or working within a multidisciplinary rehabilitation department preferred.
  • Strong leadership, communication, organizational, and team‑development skills.
  • Ability to balance department leadership responsibilities with direct patient care.
