- Complete Start of Care (SOC) admissions
, delivering thorough in-home assessments and accurate OASIS documentation. - Develop and initiate individualized care plans
, ensuring each patient is set up for safe and effective home health services. - Identify risks, gaps, and service needs early
, coordinating the right disciplines and resources from the start. - Communicate clearly with providers, caregivers, and internal teams to ensure alignment and continuity of care.
- Set the tone for quality, compliance, and patient experience from the very first visit.
- Active Minnesota RN license with strong clinical foundation.
- Proven experience with home health admissions and OASIS documentation
. - Ability to think critically, prioritize quickly, and make confident decisions independently
. - Strong communication skills with a focus on patient education and care coordination
. - Reliable transportation and comfort working in a field-based environment

- Mileage reimbursement provided.
