Job Summary
Full‑time Clinician (Nurse Practitioner or Physician Assistant) who provides in‑home health evaluations, traveling up to 100% of the time within a defined coverage area. The position involves performing medication reviews, physical assessments, and, when ordered, diagnostic tests such as diabetic eye exams and spirometry. Clinicians use an iPad and an intuitive workflow and are not required to prescribe medication or order lab tests.
Responsibilities- Travel locally and within licensed states to conduct home visits lasting approximately 45 minutes.
- Build and maintain trusted relationships with patients to improve health outcomes.
- Perform medication and medical history reviews, physical evaluations, and diagnostic tests as ordered.
- Use an iPad and clinical workflow tools during each visit.
- Conduct virtual visits when needed.
- Provide Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).
- Active, unrestricted license(s) in the required coverage area(s).
- Completion of 2080 hours of post‑graduate collaborative practice hours as an NP or PA in an integrated clinical care setting.
- Board certification.
- Multi‑state licenses (preferred).

The typical pay range for this role is: $87,035.00 - $. The actual base salary offer will depend on experience, education, geography and other relevant factors.
