In-Home Health - Nurse Practitioner or Physician Assistant; Part Time Marshall, MN

Signify Health, part of CVS Health®, is seeking a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with patients in their homes and bridging gaps in care.

• Conduct 45‑minute in‑home visits, including medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by the health plan.
• Use an iPad and intuitive clinical workflow to document each evaluation; prescribing, ordering lab tests or altering current treatment regimens is not required.
• Travel locally and within licensed states as needed; conduct virtual visits when appropriate.
• Provide all Signify Health services indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, etc.
• Build trusted relationships with patients and connect them to appropriate care.

• Active, unrestricted license(s) in coverage area(s).
• Completion of 2,080 hours of post‑graduate collaborative practice hours as an NP or PA in an integrated clinical care setting (required, and filing of the appropriate attestation with the state of Minnesota).
• Board certification (required).

• Multi‑state licenses.

The typical pay range for this role is $41.85‑$90.13 per hour, depending on experience, education, geography and other factors.

As an in‑home clinician, the primary workplace is the private residence of health plan members. Candidates must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants.
