Position

Part Time Clinician (Nurse Practitioner or Physician Assistant)

Provide in‑home health evaluations, engaging clients in the comfort of their own homes and bridging gaps in care. Each visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and optional diagnostic tests as ordered by the client's health plan.

• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
• Be flexible to travel locally and within licensed states, as needed.
• Conduct virtual visits as needed.
• Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, and more.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Able to cover the primary location:
  Saint Cloud and surrounding area.
• NP or PA credential required.

Primary workplace: private residence of members. Must be able to travel locally and within licensed states and perform duties safely in diverse home environments.

Anticipated weekly hours: 40. Pay range: $41.85 - $90.13 per hour (base rate). Part‑time schedule.
