In-Home Health - Nurse Practitioner or Physician Assistant Grand Forks, MN

Signify Health, part of CVS Health®, is seeking a Full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations for individuals in their own homes.

• Conduct approximately 45‑minute visits that include a medication and medical history review, a physical evaluation, and diagnostic tests (e.g., diabetic eye exams, spirometry) when ordered by the person’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation.
• Provide health evaluations and related services, such as diagnostic and preventive services products (spirometry, DEE, etc.), while not prescribing medication, ordering lab tests, or altering existing treatment regimens.
• Be flexible to travel locally and within licensed states as needed and conduct virtual visits when required.
• Join the national network of clinicians, visiting millions of people nationwide and connecting them to appropriate care based on each individual’s needs.
• Maintain a patient‑centered approach, building trusted relationships to improve health outcomes.

The primary workplace is the private residence of health plan members. Candidates must be able to work safely in homes that may contain pets, secondhand smoke, and other potential airborne allergens or irritants.

• Active, unrestricted license(s) in coverage area(s).
• Completion of 2,080 hours of post‑graduate collaborative practice hours as an NP or PA in an integrated clinical care setting (required), along with filing the appropriate attestation with the state of Minnesota.
• Board certification (required).
• Multi‑state licenses (preferred).
• Anticipated weekly hours: 40 hours.

The typical pay range for this role is $87,035.00 – $, depending on experience, education, geography, and other factors.
