Job Title: Registered Nurse (RN) Case Manager – Home Health

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week

Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

• Manage a home health caseload across homes and community settings

• Complete OASIS assessments and individualized care plans
  .
• Provide skilled nursing care, education, and coordination
  .
• Adjust care plans based on patient progress and needs
  .
• Collaborate with physicians and interdisciplinary teams.
• Maintain accurate, timely documentation
  .

• 3+ years of RN experience, home health preferred.
• Strong clinical judgment and care planning skills.
• Ability to work independently in a field-based role.
